We must be able to reach a responsible adult at all times while your child is on site, so we need a second number as well as yours.

Children are only released to a parent or a person named here. Tell the office in advance if someone else will collect.

GP Details

Fee Payment

The fee is £50 per child, per calendar month, from September to July. Please set up a standing order. Fees are due regardless of breaks; those breaks have already been factored in.

I/We confirm that, as far as I am/we are aware, the information on this form is complete and accurate. If any information is incorrect, the Academy may review the application and withdraw the child's place. I/We take responsibility for our child's behaviour, progress, attitude and punctuality, and will follow the conditions of admission and the rules and policies of Quba Academy set out on theParent Information page. I/We have told the Academy about any special educational needs and medical conditions.
